Library Cards
Borrowing privileges will be extended to those patrons holding a valid Louisville Public Library card and who are in good standing. Holders of unexpired cards from other SLIC libraries have full borrowing privileges at Louisville Public Library. Therefore, a Louisville card will not be issued to patrons who already have a card from another SLIC agency.
Patrons can apply for one of the following types of library cards:
- Unrestricted Card
An unrestricted library card can be issued to persons 14 years of age or above. This card gives the holder borrowing privileges to all circulating materials but subject to circulation limits as defined in the circulation policy. For patrons 18 and above, the card application form will be processed when proof of residence is provided. The following will be accepted as proof of residence: Driver's License, State Identification Card, or other documents showing current address. A signature on the application form indicates the patron is assuming responsibility for borrowed materials. In addition, minors who do not currently have a card in the computer system would need to show proof of age. For minor patrons with unrestricted cards, the laws of the State of Ohio apply regarding responsibility for borrowed materials. There is no charge for the first card. There is a $3.00 replacement fee for a lost card. A library card is good for a period of three years. When it expires, this same card can be renewed for another three-year period. A card in poor condition will be replaced without charge. - Restricted Card
A restricted library card is issued to patrons under the age of 14. This card is valid until a choice is made to apply for an unrestricted card. A restricted card is subject to borrowing limits defined in the circulation policy. A parent or legal guardian's signature is required. The parent or legal guardian thereby assumes responsibility for borrowed materials. Proof of residence of the parent or legal guardian is required. The following will be accepted as proof of residence: Driver's License, State Identification Card, or other documents showing current address. There is no charge for the first card. There is a $1.00 replacement fee for a lost card. A library card is good for a period of three years. When it expires, this same card can be renewed for another three-year period. A card in poor condition will be replaced without charge. - Teacher Card
A teacher library card will be issued to patrons who teach in the Louisville School District, who teach in an educational facility outside of the school district, or who are home educators. Teachers borrowing materials from the Stark Libraries Information Consortium (SLIC) need have only one card issued by one of the consortium members. This card is issued in addition to a personal library card held by the teacher. Teacher cards are issued at no cost to the patron. The teacher card must be renewed annually in September. A $1.00 fee will be charged for any lost or damaged card. Fines will be charged for overdue materials in line with the fine structure currently in effect at the library. This card gives the holder borrowing privileges for all circulating materials subject to the following limitations:- Materials can be checked out for 6 weeks of instructional use and cannot be renewed.
- Most juvenile AV and adult nonfiction AV materials can be checked out on the teacher card.
- Theme kits, art prints, adult fiction videos and DVDs must be checked out on the teacher's personal card for the normal loan period.
- The number of items checked out on a teacher card is at the discretion of the library in order to meet the needs of all patrons. Some materials may be restricted due to seasonal or high demand.
- Teachers will not be charged for up to 3 lost or damaged items per year. Charges will apply for all items beyond that limit. in addition, teacher's will be charged for all items whose cost exceeds $40.
Fee for replacement of lost library cards will be:
- Adult: $3.00
- Children's: $1.00
